By registering for or attending the Mind & Match Mental Wellness Seminar, I acknowledge and agree to the following:
1. Educational Purpose Only
This webinar is provided for general educational and informational purposes only. It is intended to support awareness of mental wellness topics related to youth sports, performance pressure, resilience, emotional regulation, injury recovery, and athlete development.
2. Not Therapy or Medical Advice
Participation in this webinar does not create a therapist-patient, physician-patient, or other clinical treatment relationship with Advanced Behavioral Health Services or any presenter. The information shared during the webinar is not a substitute for professional mental health treatment, medical care, psychological evaluation, diagnosis, or individualized clinical advice.
3. No Individualized Recommendations
The presenters may discuss general strategies and examples, but they will not provide personalized mental health advice, diagnosis, treatment recommendations, medication recommendations, or crisis intervention during the webinar. 6. HIPAA and Protected Health Information
Advanced Behavioral Health Services takes privacy seriously. However, attendees should understand that this webinar is educational and is not a clinical appointment. Attendees should not submit or disclose protected health information, personal medical history, or confidential clinical details through the webinar chat, Q&A, registration form, or discussion unless specifically directed through an appropriate clinical communication channel. The HIPAA Privacy Rule protects individually identifiable health information held by covered entities and requires safeguards for protected health information.
7. Parent/Guardian Consent for Minors
For attendees under age 18, a parent or legal guardian should complete registration and consent to the minor’s participation. Parents/guardians are encouraged to attend with their child when appropriate, especially because the webinar may discuss topics such as anxiety, emotional stress, injury recovery, and mental wellness.
8. Recording and Materials
The webinar may be recorded for internal, educational, or future training purposes. Attendees will be notified if recording is taking place. Attendees agree to participate respectfully. Harassing, discriminatory, disruptive, or inappropriate comments or behavior may result in removal from the webinar.

Advanced Behavioral Health Services is not responsible for technical difficulties, internet connection issues, device problems, platform outages, or other technology-related interruptions that may affect access to the webinar.
11. No Guarantee of Results
